How to Request Special Services Intake Packet/Request to be Considered for Accessibility and Academic Accommodations

​In order to receive special accommodations, such as extended time on tests or note takers for college-level classes, a student needs to register with the Department of Special Services and be approved for Academic Accommodations. Students must complete the Special Services Intake Packet/Request to be Considered for Accessibility and Academic Accommodations​ and submit supporting documentation that specifies their disability. 

To receive a Special Services Intake Packet/Request to be Considered for Accessibility and Academic Accommodations via email, complete the electronic form below and make certain to include your email address.

If you do not include your email address in the electronic form, the Special Services Intake Packet/Request to be Considered for Accessibility and Academic Accommodations will be sent to you via US Postal mail. You can also request a Special Services Intake Packet/Request to be Considered for Accessibility and Academic Accommodations​ by calling the Special Services Office at (856)-415-2265.
