If you need to make changes to your student record, please submit the appropriate form below to the Cumberland One Stop Enrollment Services Office or [email protected] from your student e-mail account. Some forms may be available to submit electronically via your Self Service account.

The Cumberland Campus is now able to send and receive transcripts electronically through National Student Clearinghouse! Please select the Transcript Request for more information. For immediate needs, unofficial transcripts can be obtained by logging into your Self Service account.
Official transcript evaluations for incoming transfer students will occur on a weekly basis. Please work with your advisor to obtain approval to override a pre-req based on an unofficial transcript.
Enrollment Verification Request can be submitted to the Admissions Office. Enrollment verifications can be sent electronically. A PDF copy of the letter will be e-mailed back to the student's e-mail address for immediate requests. For requests that need to be mailed or include attachments, they will be processed on a weekly basis.
